Care & Domestic Assistance/Activity of Daily Living Reports
The scope of a Medico Legal “Care” Report includes the claimants reasonable and necessary need for past and future care, domestic assistance, professional services, home modifications and equipment. The level of detail is determined by the clients injuries.
As a guide our standard report includes the following information:
- History - a comprehensive overview of the claimants injury, treatments, their pre injury roles and recovery to date
- Assessment - of the client’s physical, cognitive and psychological capacity to perform tasks of daily living - personal care and domestic work, community access, carer roles, employment, leisure and recreation
- Past care – a detailed appraisal of past gratuitous care, carefully cross referenced to pre injury roles and responsibilities
- Current and Future Care – detailed requirements for personal care, domestic assistance and childcare taking into consideration various future scenarios e.g. living alone, future surgery etc
- Professional Services - requirements for ongoing professional services such as Occupational Therapy, Physiotherapy, Pain Management programmes, Home Modifications, Funds Management, etc
- Equipment - comprehensive information of all recommended equipment including photo, rationale for need, costings, local supplier and replacement timelines..
- Other Services - recommendations for non professional services such as handyman, gardeners, builders for home modifications, removalists etc.
- Pricing and Past care hours: a supplementary report containing a spreadsheet summary of gratuitous past care from the date of injury until the assessment date as well as the commercial weekly costs of all recommended Care and Domestic service
Our typical turnaround sees us providing an assessment appointment within 14 days of instruction (subject to client availability) and delivering a completed expert report within a further 21 days. Urgent report deadlines can often be met, please contact us for details.
